Beman 8682840301 Revert "Create first merge point for Git" 
This reverts commit 01af50eb2a, reversing
changes made to b3cb84d8d0.

The reverted commit had the effect of backing out several previously committed fixes. Possibly operator error?

Beman Dawes e3b6783e6c Eliminate default_format from header. This eliminates initialization issues at the cost of some additional overloads. Docs to follow. Suggested by Rob. This change was made simpler by combining and reorganizing so there are now only two source files.
